Working Permit

Obtaining a Work Permit at Tohickon Middle School:

1. Work permits are available for pick up during the school day in the main office.
2. Please print and fill out the work permit application.  Include as much information as possible. You only need to fill out one half sheet. Must include a parent/guardian signature.
3. Please return completed application with copy of either birth certificate or passport to Mrs. Chmiel.
